Most over-testing comes from ordering by panel instead of by question. Decide what you need to know first, and the instrument picks itself — along with the three you did not need to run. Order by question, not by panel The usual sequence is backwards. A practitioner has a complicated patient, feels the pull to be thorough, and orders a stool test, a breath test, a hormone panel and a nutrient panel at once. Two thousand dollars later there is a folder of results and no more c

A bout of food poisoning years ago can leave behind an autoimmune injury to the gut's pacemaker cells. That injury impairs motility permanently, and impaired motility is why the SIBO keeps coming back no matter how well you clear it. The question it answers Most IBS diagnoses are arrived at by exclusion. Everything else was ruled out, nothing was found, and the patient leaves with a label that describes their symptoms back to them and explains nothing. They have usually also

A large share of the world carries H. pylori and most of them will never have a problem from it. So a positive result is the beginning of the question, not the answer — and the virulence factors underneath it are where the answer actually lives. Presence is not the question H. pylori is one of the most widely carried organisms in human beings. Estimates vary by region and by population, but across the world it is carried by a very large number of people, most of whom never de

When a patient stops a proton pump inhibitor and the reflux comes back worse than before, that is not proof they needed the drug. It is a predictable physiological rebound, it has a timeline, and it is the reason most attempts to stop fail in the first fortnight. Rebound acid hypersecretion The drug does not just suppress acid. It changes the machinery that makes it. Acid secretion is regulated by gastrin, and gastrin release is held in check by the acid itself — a negative f

You can support every phase of estrogen detoxification correctly and still watch the levels climb back, because the last step does not happen in the liver. It happens in the gut, and whether it finishes depends on the bacteria living there. Where estrogen actually goes Most practitioners can recite the first two phases. The failure point is after them. Phase I hydroxylates estrogen into its metabolites — 2-OH, 4-OH and 16-OH — via cytochrome P450. Which pathway dominates matt

A patient with low cortisol and a patient who is clearing cortisol quickly look identical on a saliva test and need opposite things. Measuring only the free fraction is why so many exhausted patients get the same protocol and only some of them improve. One number isn't enough Cortisol is not a quantity sitting still in the blood. It is produced, bound, freed, used, converted and cleared, and a single measurement catches one moment of one of those processes. Saliva testing — w

The markers that first suggest mold exposure are usually already sitting in labs you ordered for something else. They look non-specific one at a time, which is exactly why the pattern gets missed for years. Why it gets suspected late Mold-exposed patients do not arrive saying they have been in a water-damaged building. They arrive with fatigue, brain fog, air hunger, unexplained weight gain, light sensitivity, static shocks, headaches and anxiety — a list long enough that it

A GI Map comes back with eleven things in red and the patient is sitting in front of you on Thursday. The hard part was never understanding the markers — it is deciding which one you touch first, and which flags you are supposed to ignore. The report is not a priority list The flags tell you what is outside the laboratory's range. They do not tell you what is driving the patient's symptoms, and they are not ordered by how much any of it matters. That sounds obvious written do

A conventional reference range is a statistical description of the people who got tested, not a statement about health. Knowing how it is built changes what you do with a result that comes back "normal" in a patient who clearly is not. Where the range comes from A reference interval is the middle 95% of results from a reference population. That is the whole definition, and everything that bothers us about it follows from those two words. Middle 95% means the top and bottom 2.

A negative SIBO breath test in a symptomatic patient is more often the wrong substrate, an incomplete gas panel, or a rise read outside the diagnostic window than a true negative. Here is how to choose the test and read what comes back. Substrate: lactulose or glucose The two substrates don't test the same bowel, so the one you order decides what you are able to find. Glucose is absorbed in the proximal small intestine — roughly the first two to three feet. Bacteria living pa
